Total GSA Forms 3080 received during the period was 3,776. This is the number of carrier evaluations returned from relocating employees and GBL Issuing Officers. Except in certain cases, the evaluations used involved shipments having both pickup and delivery dates in Calendar Year 1996. The cutoff for inclusion of CY 1996 evaluations was the end of February 1997.

Rate of return was 29.4%. This is the percentage of return of GSA Forms 3080 to the total number of shipments reported by a carrier. In this case, the 3,776 GSA Forms 3080 represent 29.4% of the total of 12,840 shipments reported to GSA. There are two sets of evaluation elements on the GSA Form 3080. The first sets out responses by the relocated employee. The second set are responses by the GBL Issuing Officer for the shipment of that relocated employee.
